Cave Diver Forms

Diving the cave system at Camp Indian Springs requires the use of a guide. Print out, complete and return these forms to your guide if you want to dive Camp Indian Springs:

NOTE: Diving will begin after March 10th, 2007

• Diver Application
  In addition to the diver application you will need to provide copies of:
• Full Cave card
• Trimix certification card(s)
• Proof of DAN insurance of equivalent
• Proof of 100 cave dives after full cave
• Diver Waiver
• General release/liability waiver
• Statement of understanding
